Job Description
This position is for a Graduate Physical Therapist within Western CT Home Care Inc. The therapist will provide therapeutic services to evaluate, diagnose, and rehabilitate patients, collaborating with the physician and caregiver to develop an individualized treatment plan. Responsibilities include functional assessment, communication with the physician, and participation in case management. New graduates will be mentored and supported through competency clarification.
Requirements
- Doctorate in Physical Therapy
- Current State of Connecticut Physical Therapist License
- Current Driver's License
