Requirements

• Perform physical therapy assessment
• Identify patient and family needs for home health services
• Provide appropriate treatment
• Record evaluation data and client responses
• Participate in case management conferences
• Provide counseling and education to patients
• Supervise care by PTA and home healt aides
• Ensure compliance with agency requirements
• Perform other duties as assigned
