Physical Therapist

The Physical Therapist provides services to identify, prevent, remediate, and rehabilitate acute or prolonged physical dysfunction or pain. This involves performing comprehensive assessments, providing treatment plans, and coordinating care with other healthcare professionals. The therapist will also be responsible for educating patients and family members.

Requirements

• Perform physical therapy assessments including functional limitations, mobility status, endurance status, ADL status.
• Identify patient and family needs for other home health services.
• Provide appropriate treatment.
• Record all evaluation data and treatment responses.
• Participate in case management conferences.
• Provide counseling and education.
• Supervise care and arrange for necessary equipment.
• Participate in quality assurance and performance improvement programs.
• Perform other duties as assigned.
