Registered Nurse (RN) provides high-quality nursing services to patients in the comfort of their own homes, helping them manage their care and regain independence.
Requirements
- Provide in-home nursing visits according to the physician’s plan of care
- Assess patients’ health status, including condition, history, and home environment
- Develop and update individualized care plans based on patient needs
- Administer medications and treatments as prescribed
- Educate patients and caregivers on disease management and home care
- Monitor patient progress and communicate changes to the physician and care team
- Document visits and care in a timely manner
- Coordinate care with therapists, aides, and other team members
- Serve as case manager for assigned patients and support discharge planning
