Registered Nurse (RN) For Home Healthcare

Physicians Choice Home Health is hiring a Registered Nurse (RN) to provide high-quality nursing services to patients in the comfort of their own homes, helping them manage their care and regain independence. The role involves providing in-home nursing visits, assessing patients' health status, developing and updating care plans, administering medications, educating patients and caregivers, and coordinating care with therapists and other team members.

Requirements

• Provide in-home nursing visits according to the physician’s plan of care
• Assess patients’ health status, including condition, history, and home environment
• Develop and update individualized care plans based on patient needs
• Administer medications and treatments as prescribed
• Educate patients and caregivers on disease management and home care
• Monitor patient progress and communicate changes to the physician and care team
• Document visits and care in a timely manner
• Coordinate care with therapists, aides, and other team members
• Serve as case manager for assigned patients and support discharge planning
