Requirements

• Conduct initial patient evaluations and ongoing reassessments to meet nursing care needs.
• Manage caseload, including assessment, planning, coordination, implementation, and evaluation of patient care plans.
• Perform hospice-specific assessments, including home safety evaluations and availability of support systems and community resources.
