Clinical Care Transition Educator / Registered Nurse

Evaluate, educate, and train patients, caregivers, and facility staff about Option Care Health services and products for successful transition to in-home or alternative site care. Conduct patient assessments and evaluations to determine patient viability to join Option Care Health care delivery model. Provide patient and caregiver education, gather pre-admission and insurance information, and facilitate initiation of services to support safe and effective care at home.

Requirements

• Active and unrestricted Registered Nurse (RN) license required.
• Minimum of 2 years of experience in the healthcare industry.
• Basic PC skills: Able to competently use internet, email, Microsoft Word, Microsoft Excel, Microsoft PowerPoint
