Pacific PACE LLC is seeking an Occupational Therapist to join their team. The role involves providing compassionate, coordinated care to older adults, conducting comprehensive assessments, and developing personalized care plans. The OT will work as part of an interdisciplinary team to improve the health and quality of life for participants.
Requirements
- Provide Occupational Therapy treatment and interventions to PACE participants
- Conduct comprehensive assessments of participants to determine their functional abilities and needs
- Actively participate in the interdisciplinary team by offering clinical expertise in team planning, problem-solving, and processes
- Develop and implement personalized care plans aimed at improving or maintaining participants’ independence and quality of life
- Guide participants in therapeutic, creative, and self-care activities to enhance their functional abilities
- Review participants’ progress and adapt treatment plans as needed
- Provide training, guidance, and supervision to Occupational Therapy Assistants (COTA) or Personal Care Assistants, and participant/family members in carrying out the Functional Maintenance Program
