Job Description
Palm Primary Care is seeking a Primary Care Physician to provide comprehensive primary care services to adult and senior population within the Medicare Advantage/Medicare/Medicaid model. The ideal candidate will have a medical degree, board certification in Family Medicine or Internal Medicine, and 2-3 years of experience providing care to Medicare and/or Medicare Advantage patients.
Requirements
- Medical Degree (M.D., D.O.) from an accredited institution
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Active, unrestricted Texas Medical License
- Current DEA license in good standing
- Minimum 2-3 years' experience providing care to Medicare and/or Medicare Advantage patients
- Experience working in a value-based care model, focused on outcomes, quality metrics, and patient satisfaction
- Strong understanding of HEDIS, STAR ratings, risk adjustment, and preventative care measures
- Demonstrated ability to manage chronic conditions and promote population health in a senior patient population
- Excellent communication, collaboration, and interpersonal skills for a multidisciplinary team environment
- Comfortable using EMR systems (eCW a plus)
