Job Description
Join PATH as a Case Manager at the Villas On The Park location and help individuals experiencing homelessness break the cycle of homelessness. As part of the Site Based Housing team, you will conduct intake and individualized needs assessments, develop Individualized Service Plans, and provide referrals and linkages to resources.
