Geriatric Nurse Practitioner - (Edmonds-Bothell Area)

Pathway Geriatrics is a nurse practitioner-owned medical practice headquartered in Redmond, WA, that provides in-home palliative primary care services for homebound older adults. We are looking for an experienced Geriatric Nurse Practitioner to serve as a palliative primary care provider in the Edmonds/Lynnwood/Bothell region.

Requirements

• Monitor disease progression and consider when clients should transition to hospice services
• Manage end-of-life care for clients
• Work with local home health care teams for skilled nursing, physical and occupational therapy, speech therapy, and psych nursing
• Provide specific orders and permissions for Adult Family Home inspection requirements
• Weigh the risks and benefits of applying antidepressants, antipsychotics, anticonvulsants, and other medications to manage dementia behaviors
