RN Case Manager

Pathways Hospice is seeking a RN Case Manager to provide holistic care to patients and families. The role involves leading and coordinating care, building meaningful connections, and collaborating with a supportive team. Requirements

• Graduate of an accredited school of nursing (BSN preferred)
• 1–2 years of experience in hospice, palliative care, home health, med-surg, oncology, public health, or acute care
• Current Colorado RN license and CPR certification
• Valid Colorado driver’s license, auto insurance, and ability to travel daily
• Ability to lift up to 35 lbs
