Job Description
Hinds Lifecare is seeking a Full Time RN Case Manager to join our team. The RN Case Manager provides skilled palliative and end-of-life care in the community setting. As a member of the Interdisciplinary Team, the RN Case Manager facilitates the development of a collaborative interdisciplinary plan and ensures the overall coordination, collaboration and medical management of hospice services for patients and their significant others.
Requirements
- Completed a professional nursing program accredited by the National League for Nursing
- One year experience in acute care facility or community health agency (hospice experience a plus)
- Current RN Licensure and CPR Certification
- Valid driver’s license, auto insurance, reliable transportation and a willingness to use personal vehicle in the course of employment
- Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 35 lbs.)
