Job Description
Hinds Lifecare is seeking a Full Time RN Case Manager to join their team. The RN Case Manager provides skilled palliative and end-of-life care in the community setting, facilitating interdisciplinary plans and ensuring coordination of hospice services.
Requirements
- Completed a professional nursing program accredited by the National League for Nursing
- One year experience in acute care facility or community health agency
- Current RN Licensure and CPR Certification
- Valid driver’s license, auto insurance, reliable transportation, and a willingness to use personal vehicle in the course of employment
