Job Description
Pathways Hospice has an immediate opening for an RN Clinical Liaison. The role involves building effective relationships with hospitals, facilities, and physicians to promote communication and problem-solving, and providing assessment, supportive care, and education to prospective patients and families regarding hospice services.
Requirements
- Graduate of an accredited school of nursing; BSN preferred.
- 1-2 years of experience in hospice and/or palliative care, medical/surgical, public health, oncology, acute care or home health.
- Current Colorado RN licensure. Valid Colorado State driver’s license and auto insurance. Current CPR certification.
