Admissions RN

Pathways Hospice is seeking an RN Admissions for a full-time position in Fort Collins, CO. The role involves assessing prospective patients and families, providing supportive care and education, and determining hospice appropriateness. The successful candidate will have a strong nursing background, preferably in hospice, palliative care, or home health, and a current Colorado RN licensure.

Requirements

• Graduate of an accredited school of nursing
• 1-2 years of experience in hospice palliative and/or home health care, medical/surgical, public health, oncology, acute care or home health
• Current Colorado RN licensure
• Current CPR certification
• Valid Colorado State driver’s license and auto insurance
