Home Care RN Case Manager | PRN

The Home Care RN Case Manager coordinates and directs the home care patient's care in their home environment based on individual needs. They are responsible for evaluating the patient for appropriateness of home care needs, developing the plan of care, and monitoring the patient and caregivers response to the plan.

Requirements

• Graduate of an accredited Diploma of Nursing Program or Associate's Degree in Nursing
• Current Texas state license in appropriate discipline
• Current American Heart Association CPR
• Current Texas state driver's license
• Current auto insurance
• One year experience in a healthcare setting in respective discipline
