The FIR Case Manager works directly with participants who are court-stipulated for treatment services with oversight from the Adult Probation and Parole departments. The primary responsibilities are to ensure that services are provided to participants and documented according to DAP, CBH, Forensic Services, and PHMC requirements.
Requirements
- Provide case management services to a chemically dependent forensic population
- Establish and maintain service productivity goals
- Document service provision within 24 hours
- Participate in monthly team meetings and scheduled case conferencing with Probation
- Collaborate and attend meetings with Probation/Parole, treatment providers, and other collaborating agencies
- Maintain participant records in accordance with agency fee-for-service standards, managed care, and licensing requirements
