Job Description
The Recovery Support Outreach Case Manager provides individualized case management services to individuals in recovery from substance use disorders. This role involves assessing client needs, developing recovery plans, connecting clients with resources, and offering ongoing support to promote long-term stability and well-being.
Requirements
- Bachelor's degree in social work, psychology, counseling, or a related field
- Minimum of two years of experience in case management, behavioral health, or recovery support services
- Experience working with diverse populations and knowledge of trauma-informed care principles
