Job Description
Lead Medical Case Manager oversees care coordination and provides support for clients diagnosed with HIV/AIDS at the Care Clinic. The role reports to the HIV Program Manager and is responsible for maintaining accurate and timely documentation of all client contacts and coordinating Psychosocial Support Groups.
Requirements
- One-two years of experience in HIV/AIDS case management services or related field
- Bachelor's Degree in Social Work, Human Services, Sociology, Psychology, Nursing, Public Health, or related field
- Bilingual/bi-cultural a plus
- Knowledge of community resources helpful
- Strong interpersonal and organizational skills
