Job Description
Pinnacle Services is looking for a Case Manager to join their Hennepin and Scott County team. The successful candidate will have a bachelor's degree in social work or a related field and will work with individuals with intellectual, developmental, or behavioral disabilities. The role involves assessing client needs, coordinating resources, and developing service plans.
Requirements
- Bachelor's degree in social work or a related field
- One year of full-time experience as a social worker/case manager/care coordinator
- Valid driver's license and acceptable driving record
- Successful completion of local and/or state background checks
