Requirements

• Provide orientation to new patients
• Conduct Intakes, Discharge Planning, Initial Assessments, and Initial Treatment Plans
• Administer biopsychosocial assessments
• Develop and implement individualized Treatment Plans
• Monitor and document patient progress
• Comply with Medical Q.A (Quality Assurance) and Clinical Risk Management Policies
• Provide counseling services, including individual and group counseling
• Document counseling entries
• Engage in Community Relations and Outreach efforts
• Attend team meetings and complete training courses
• Collect research data when requested
• Other duties as assigned
