Requirements

• Perform initial and ongoing patient assessments
• Develop and implement tailored treatment plans
• Collaborate with the interdisciplinary care team
• Provide education and support to patients and families/caregivers
• Maintain accurate clinical records
• Supervise Physical Therapy Assistants
• Participate in in-services and meetings
• Identify patient and family/caregiver needs for additional services
• Confirm daily visit schedules
• Communicate patient progress and changes with physicians and care teams
• Document all visits and progress notes
