Job Description
Health Coordinator supports Pioneer Human Services' mission by coordinating and monitoring client physical and behavioral health services within residential treatment facilities. The role assists in ensuring continuity of care by facilitating communication between clients, nursing staff, medical and behavioral health providers, pharmacy services, and program staff.
Requirements
- High School Diploma or Equivalent
- One (1) year of experience working in a caregiving, human services, behavioral health, or social services setting, or experience supporting marginalized or vulnerable populations
- Must apply for Agency Affiliated Counselor registration within 30 days of hire and obtain and maintain active registration within 90 days of hire in accordance with WAC 246-810-015; RCW 18.19.210
- First Aid/CPR certification within 90 days of hire
