Health Coordinator - ON-CALL

Pioneer Human Services is looking for a Health Coordinator (On-Call) to support client physical and behavioral health services within residential treatment facilities. The role assists in ensuring continuity of care by coordinating communication between clients, nursing staff, medical and behavioral health providers, pharmacy services, and program staff.

Requirements

• High School Diploma or Equivalent
• One (1) year of experience working in a caregiving, human services, behavioral health, or social services setting
• First Aid/CPR certification
