Requirements
- MD or DO degree
- Active Illinois medical license with controlled substance authority
- DEA registration (Illinois address)
- ACLS and BLS certification
- Board certification in Family Medicine, Internal Medicine, or Obstetrics & Gynecology
- Minimum 5 years post-residency clinical experience
- Leadership experience
