The Postgraduate Center for Mental Heal (PCMH) has been a leader in community‑based mental health services since 1945, operating 22 apartment buildings, six ambulatory offices, and 500 community‑leased units that house 1,500 tenants and clients. The Care Coordinator is responsible for addressing member needs, providing care plan updates, and conducting outreach to members in between visits. They advocate and support members, engaging with community agencies/health care providers, and others on the member’s behalf to ensure access to necessary services that promote wellness, self-management, and reduce emergency room visits and/or hospitalizations.
