Job Description
As a Care Coordinator, you will address member needs, provide care plan updates, and conduct outreach to members. You will provide care coordination to NYC Medicaid beneficiaries with chronic health and/or behavioral health disorders using a Health Home service model.
Requirements
- HS Diploma/GED
- 2 years of related human services experience
- Experience in providing direct services to individuals with chronic health and/or behavioral health disorders
