Job Description
Join our team as a Service Coordinator Clinician to make a meaningful impact on the lives of individuals with chronic illnesses, co-morbidities, and disabilities. Conduct assessments, identify member needs, and develop person-centered service plans in a collaborative and supportive environment.
Requirements
- Current, unrestricted RN, LSW, LCSW, or LMHC license in applicable state(s) required
- Minimum of 2 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role
- Strong communication and interpersonal skills
- Ability to work effectively as part of an interdisciplinary team
- Compassionate and patient-centered approach to care

- Pay: $82,000-$85,000 depending on licensure and experience
