Job Description
Case Manager position available at Leona Blanche House Residence, providing housing for formerly homeless single adults with chronic mental illness. The Case Manager will work as a key member of an interdisciplinary team, providing direct client service, assessments, and individualized service plans.
Requirements
- Bachelor's Degree in Social Work or other Human Services related field
- Significant experience working with the mentally ill and or dually-diagnosed, homeless population
- Bilingual in Spanish preferred
