Case Manager

We are seeking a Case Manager to join our team at Ana's Place, a 108-bed emergency shelter for homeless men with mental illness and/or substance abuse. The Case Manager will conduct intake and assessments, provide case management and counseling services, and assist clients in obtaining financial entitlements and housing.

• Maintain accurate client documentation
• Provide escort services as needed
