Job Description
At Qualicare Home Health, we are committed to providing compassionate, high-quality care that enhances the lives of our clients. We are seeking a dedicated Physical Therapist (PT) to join our team and deliver skilled in-home therapy services, design personalized treatment plans, and work collaboratively with caregivers and clinical staff to help clients restore mobility, regain independence, and improve their overall quality of life.
Requirements
- Current California Physical Therapist (PT) license.
- Minimum of 1 year of experience in a Home Health setting, including use of OASIS documentation.
- Proficiency with EMR systems (WellSky/Kinnser preferred).
- Strong interpersonal and communication skills.
- Ability to work independently while maintaining strong collaboration with interdisciplinary teams.
- Valid driver’s license and reliable transportation.
