The Care Navigator is a hybrid role combining in-person engagement with remote telehealth and administrative support to ensure clients receive timely, person-centered care.
Requirements
- High School Diploma or equivalent
- Associate’s or Bachelor’s Degree in healthcare, social services, or a related field from an accredited school/university
- Knowledge of community resources, health systems, and behavioral health navigation
- Experience in behavioral health, case management, or community navigation
- Must be at least 21 years of age
- Must have strong leadership and organizational skills
- Must maintain a valid Driver’s License and insurability
- Must have excellent communication and conflict-resolution skills
