Care Navigator

The Care Navigator is a hybrid role combining in-person engagement with remote telehealth and administrative support to ensure clients receive timely, person-centered care.

Requirements

• High School Diploma or equivalent
• Associate’s or Bachelor’s Degree in healthcare, social services, or a related field from an accredited school/university
• Knowledge of community resources, health systems, and behavioral health navigation
• Experience in behavioral health, case management, or community navigation
• Must be at least 21 years of age
• Must have strong leadership and organizational skills
• Must maintain a valid Driver’s License and insurability
• Must have excellent communication and conflict-resolution skills
