Care Navigator

Comprehensive Healthcare is seeking a Care Navigator to work within their Hub & Spoke team to help clients and providers coordinate substance use treatment with other community services. The role involves case management, crisis intervention, and advocacy for clients.

Requirements

• Bachelor's degree in social work, psychology, or related field
• Must be eligible for registration as Agency Affiliated Counselor
• Must have valid driver's license and vehicle liability insurance
