CARE COORDINATOR - Health Homes

Comprehensive Healthcare is seeking a Care Coordinator to provide services and coordination to clients with chronic health conditions. The Care Coordinator will work with clients to develop individualized health action plans and coordinate with healthcare providers and community services.

Requirements

• Bachelor's degree in social work, psychology, or related field
• Must be eligible for registration as Agency Affiliated Counselor
• Must have valid driver's license and vehicle liability insurance
