Mental Health Targeted Case Manager

This position is responsible for the case management of up to 25 clients, creating and implementing individualized care plans, and linking families to needed resources.

Requirements

• Bachelor's Degree in a human services or related field
• One year of experience in community-based services postgraduate or a Master's Degree in a Human Services Field
• Valid Driver's License
• Successful completion of a pre-employment drug screen
• Successful completion of a background screening
• Successful completion of a TB skin test or proof of a negative chest x-ray or other documentation
