Mental Health Targeted Case Manager

This position is responsible for the case management of up to 25 clients, creating and implementing individualized care plans, and linking families to needed resources. The Targeted Case Manager will work in a fast-paced environment and be required to make quick decisions in crisis situations.

Requirements

• Bachelor's Degree in a human services or related field
• One year of experience in community-based services postgraduate or a Master's Degree in a Human Services Field
• Valid Driver's License and insurability
• Excellent communication and conflict resolution skills
• Ability to understand and relate to the needs of clients from diverse backgrounds
