Join ReDiscover’s Community Psychiatric Rehabilitation Center (CPRC) and make a difference in the lives of individuals impacted by mental illness or substance use. As a Mental Health Case Manager, you will work with a diverse caseload, providing support and resources to help clients achieve long-range outcomes of symptom management, employment, and independent living.
Requirements
- Maintain and interact with a caseload of clients
- Facilitate client achievement of long-range outcomes
- Engage difficult-to-serve clients
- Help remove barriers for clients
- Assist clients with transportation barriers
- Collect and maintain client information
- Communicate with internal multi-disciplinary team
