Job Description
SAILS Washington is dedicated to providing quality community-based support services for people of all ages and needs. We are looking for a Client Intake and Transitions Manager to connect prospective clients with our programs and ensure smooth transitions into supported living environments.
Requirements
- Experience in Supported Living Services, developmental disabilities services, case management, or related fields
- Knowledge of person-centered planning and SLS best practices
- Strong communication, relationship-building, and advocacy skills
- Ability to coordinate with multiple stakeholders, including clients, families, care teams, and financial representatives
- Excellent organizational skills, attention to detail, and the ability to manage multiple priorities
