Job Description
The Social Worker is responsible for planning, organizing, and implementing social work services to program participants and families. They are an essential member of the interdisciplinary team and vital to the integration of person-centered care.
Requirements
- Masters Degree in Social Work
- Minimum of one year of experience working with frail or elderly population
- Experience working on a multidisciplinary team preferable
- Experience documenting in an Electronic Health Record preferred
- Familiarity with Microsoft applications such as word, excel and outlook
