INCLUDE Case Manager

Rocky Mountain Human Services is seeking a Care Manager to provide respectful, individualized, and trauma-informed case management services to individuals with Intellectual and Developmental Disabilities (I/DD) and their households who are homeless or at risk of homelessness.

Requirements

• Bachelor’s degree in social work, social sciences, or related field
• Prior Case Management experience
• Valid driver’s license and proof of insurance
