Housing Case Manager

The Housing Case Manager provides ongoing support to clients in a permanent supportive housing environment, assisting with linkage to community resources and developing individual support plans to improve outcomes related to safety, wellness, and housing stability.

Requirements

• Associate’s degree and 2+ years of experience and/or lived experience in human services
• Current and valid driver's license, with a clean driving record
• Intermediate computer skills
• Excellent verbal and written communication skills
• Ability to work alone with very little supervision
