Job Description
We are searching for a Services Intake Case Manager to facilitate the initial intake process into RS EDEN Support Services programs, including establishment of eligibility and coordination of move into permanent supportive housing.
Requirements
- Screens all potential clients for eligibility for Housing Stabilization Services and supportive housing programs
- Works collaboratively with multiple team members
- Maintain electronic health records in accordance with the expectations for timely and thorough billing for services rendered
- Transportation of clients in personal vehicle within the Twin Cities metro area
