Job Description
The Case Manager, Wellness Clinic position is responsible for conducting field-based assessments, developing care plans, and providing supportive counseling and crisis intervention services to clients. The role involves working with diverse populations, including those who are homeless, limited English speakers, and individuals with multiple diagnoses.
Requirements
- BA in a health sciences or social work-related field
- At least one year of experience providing counseling, advocacy, education, and service coordination to disenfranchised communities
- Knowledge of HIV/AIDS, STIs, substance use, mental health, social service, and probation system
