Clinic LVN

As a Clinic Licensed Vocational Nurse (LVN), you will collaborate with a dedicated clinic team to support a substantial patient panel alongside Primary Care Providers (PCPs), ensuring high-quality care and positive health outcomes.

Requirements

• Assist with triage of inbound communications, including calls, faxes, emails, and instant messages from teammates, patients, and partners, prioritizing patient needs and coordinating care accordingly.
• Administer basic clinical procedures, such as wound care, vaccinations, and medication administration, within the LVN scope of practice.
• Provide on-site instruction and support to Personal Care Assistants/CNAs as needed, under the direction of the Clinic Manager and RNs.
• Manage post-appointment processes, including assisting with diagnostic orders (POC testing, labs), medication management, and referrals.
• Reinforce education for participants and their families on care plan, chronic disease management, and preventive care strategies.
• Conduct participant health assessments and assist in the development of care plans.
• Provide assistance with home care visits and participate in home safety assessments under supervision, if needed.
