Care Coordinator for Serious Mental Illness and Substance Abuse for Charlottesville, Harrisonburg, Luray, Winchester, Cumberland, and Woodbridge, VA

Sentara Health Plans is hiring a Care Coordinator to provide case management services to patients with Serious Mental Illnesses, Serious Emotional Disturbances, and Substance Abuse Disorders in Charlottesville, Harrisonburg, Luray, Winchester, Cumberland, and Woodbridge, VA. The position requires both in-person and remote assessments, with up to 90 minutes of travel time one way. The Care Coordinator will assist patients and their families with personal and environmental difficulties associated with medical conditions.

Requirements

• Degree in Health & Human Services or related fields
• Long Term Care experience (1 year)
• Health Plan experience (1 year)
• Medicaid experience (1 year)
• At least one year of mental health experience preferred
