Job Description
Sentara Health Plans is hiring a Care Coordinator in multiple locations in Virginia. The Care Coordinator will perform casework duties and provide case management services to patients and families. The position requires a degree in Health & Human Services or a related field and at least one year of long-term care experience. The role involves both in-person and remote assessments for members with serious mental illnesses and substance abuse disorders.
Requirements
