Job Description
Sentara Health Plans is hiring a Temporary Care Coordinator for the Roanoke area. The Care Coordinator performs casework duties and provides case management services to patients, families, and designated caregivers.
Requirements
- Bachelors Degree in Health & Human Services or related field
- 1 year of Long Term Care and Health Plans experience
- Nursing Home Experience (preferred)
- Case Management/Care Coordination experience (preferred)
- Experience with Community Health Resources (preferred)
