Requirements
- Bachelors Degree in Health & Human Services or related fields
- 1 year of Long Term Care and Health Plans experience
- Nursing Home Experience (Preferred)
- Case Management/Care Coordination experience (Preferred)
- Experience with Community Health Resources (Preferred)
