Community Health Worker in collaboration with healthcare team members to provide comprehensive and coordinated care to patients with chronic and/or complex illness, providing education and social support, and advocating for patients and families.

Requirements

• Bilingual in English and Spanish
• Experience working in a community-based setting
• Knowledge of medical terminology
